UHCE Advisory Board

The UHCE project promotes innovative integrated health and social care pathways, early detection of frailty, management of poly-pharmacy and prevention of falls for active and healthy ageing in European cities. The general objective is to develop, implement, and evaluate the UHC2.0 in 5 varied European cities that will act as pilot site, in order to produce and disseminate a transferable and easily implementable model for the innovative UHC2.0.

Advisory Board is established to fulfil three functions in the project. First of all it tries to support the project pilot sites in implementing innovative solutions designed in the project and help them with facing challenges related to that; secondly it provides knowledge and expertise in the field of integrated care originating from their experience; thirdly it disseminates the project results in their networks to spread the new insights in Europe and worldwide.
The Advisory Board has consisted of members from 5 different countries: municipalities of Vasteras and Eskilstuna (Sweden), the city of Antwerp (Belgium), Ministry of Health of Slovakia, the Erasmus University Medical Centre of Rotterdam, Laurens (the Netherlands) and Fudan University of Shanghai (China).
