International UHC2.0 meeting Pallini 26-27 May 2016

The Mayor of Pallini  Athanasios Zoutsos welcomed and opened  the General Assembly Meeting of the Urban Health Centers’Consortium on The  26 – 27 May, 2016

On the first day after a  brief overview of the state of affairs, each pilot site reported about its participation rate,  progress of execution and local care provision in the falls, polypharmacy, loneliness and medical  care pathways. polypharmacy. Common challenges encountered in the pilot sites were discussed, such as the use of tablets and polypharmacy,/ adherence to medication platforms when appropriate, and drop-outs. It seems that each specific age target group has its own characteristics, regardless the place of residence. Each pilot site faces obstacles based on the local profile. For example, the loneliness pathway is the most successful in Pallini, but it's the most difficult in Valencia, Rotterdam and Rijeka. On the other hand, the delivery of prevention services has problems in Pallini and not in the other pilot sites. There was also a discussion about the programming of the next steps regarding the evaluation procedure (process and scientific evaluation, data from pilots, e.g. registrations, questionnaires, focus groups), the methodology of scientific publications, how policy makers would be involved, and the development of the  toolbox. The update of the executions plans and the distribution of (T1) questionnaires were also discussed in details. Participants and professionals involved in the project would be included in the focus groups according to their local settings.

 

On the second day of the meeting there were detailed presentations about Public Health and Social Services in Pallini, experiences regarding the implementation of the loneliness and falls prevention pathway from key collaborating health and social professionals. A presentation of the Compliance to Medication/Polypharmacy e- health solution used in Pallini followed.

The Pallini Team also organized a visit to KAPI (Open Care Centers for Elderly) where the visitors had the chance to meet participants in the UHCE program and discuss their experiences about the pathways’ implementation. A visit to the Municipal Health Center and the Social Pharmacy followed.

 

The UHCE General Assembly Meeting closed with recommendations about the use of tablets, the conclusions of the meeting and expectations for coming meetings of partners.
