Implementation of the “Falls pathway” @ Rijeka

Risk analysis of falling was performed according to the generally accepted criteria. Preliminary assessment was performed with the questionnaire and measurement results done by patronage nurses. According to the results of questionnaire, 31,2% of participants experienced fall, and 56% of participants have a fear of falling. The decision making process was done according to the nurses’ opinion, experience and taking into account the points raised in the questionnaire. Activities have been offered to the participants. Personalised care is being provided.

The activities in Rijeka regarding the fall prevention were of different types:  there were advices related to the home safety assessment (lightning, slippery floors, uneven surfaces, footwear and clothing, inappropriate walking aids or assistive devices). Additional activities included the involvement of participants in the programs organized by the City of Rijeka and the Senior’s clubs (swimming pools offer three days per week for older citizens, yoga or dance groups, so this provides an opportunity for seniors to exercise and socialize).

A new intervention has been  organized throughout the UHCE project  with  exercises of elderly guided by students of Physiotherapy. In cooperation with the Faculty of health studies, 20 students /volunteers were included in group leading. The exercises were organised in 5 different locations and each location was covered by four students (one student was in charge to lead and show the exercise and the rest of the students had to  supervise  the execution).  All the equipment was provided by the UHCE project. Before this intervention begun the students  were coached to adapt the training to this specific target group. In order to monitor the progress during the training session, the students have done  additional assessments before and after the intervention phase. At the end of the intervention the users  felt a better wellbeing  and communicated to the organisers and the students their satisfaction  about the UHC project.

At the end of interventions, a final meeting was organized in City Hall with all participants from the UHCE project (elderly,

health and social care professionals, local stakeholders and media).
